Service Request Form
Town of Dover Service Request Portal
The Service Request Portal allows residents to submit service requests, report concerns, or file Open Public Records Act (OPRA) requests directly to the Town of Dover.
Your request will automatically be routed to the appropriate department based on the type of request submitted, and a tracking number will be generated for your records.
To help us respond as quickly and efficiently as possible, please provide as much detail as possible when describing your request.
Directions for Submission
- You will be asked to complete two (2) screens of information.
- The first screen contains required contact information. This information is confidential and is not transmitted with your service request.
- The second screen will display questions specific to your request type. Fields highlighted in red are required before your request can be submitted.
- For OPRA Requests, after completing the first screen, you will be directed to the second screen where only an email address is required.
- For all other service requests, simply follow the on-screen prompts and complete the required fields.
- When finished, click Submit to send your request.
OPRA Payment & Delivery Information
If you are submitting an OPRA Request, please include the following information in the Message Box, if applicable:
- Maximum Authorized Cost
- Preferred Payment Method: Cash, Check, or Money Order
- Record Copy Fees:
- Letter Size (8½" × 11"): $0.05 per page
- Legal Size (8½" × 14"): $0.07 per page
- Electronic Records: Electronic records are generally provided free of charge. However, fees may apply for the cost of media, programming, clerical or supervisory assistance, or substantial use of information technology resources.
- Delivery: Postage or delivery fees may apply depending on the delivery method requested.
- Additional Charges: Additional fees may be assessed if fulfilling the request requires extraordinary time, effort, or resources, as permitted by law.
