Facility Use Application

This application is for anyone seeking a permit to utilize the Town of Dover parks or fields.  The application must be completed in its entirety and submitted with the proper Certificate of Insurance coverage (if applicable), security deposit, and payment 30 days before the event date for approval by the Recreation Department and the Administration Office.
