Special Notes

 

Persons eligible to request a birth certificate, marriage certificate, remarriage certificate or domestic partnership certificate are the subjects, parents, legal guardian, spouse, child, grandchild or sibling of legal age. Restrictions apply.

 

Documents presented to the Registrar showing proof of relationship that are not in English must be translated by a Certified Translator.

 

If corrections are made to any certificates, there is an added charge.

 


BIRTH CERTIFICATES

 

If you were born in the Town of Dover, Morris County, and wish to obtain a copy of your birth certificate:

 

You must submit a completed application for a birth certificate accompanied by a Money Order made payable to the "Town of Dover" for $25.00 per copy.  The application form is available by clicking "application".

When requesting a birth certificate, if you have assumed your spouse's or civil union partner's surname,  you must provide a copy of your marriage or civil union certificate to link the name on your current ID to the name on your birth certificate.

If your are applying for a birth certificate for someone other than yourself, you must provide proof of relationship to the person on the record.

 

Enclose the following information with your birth certificate application:

 

  1. Telephone number.
  2. Self-addressed, stamped envelope.
  3. $25 Money Order payable to the "Town of Dover".  Please do not send cash in the mail.
  4. A photocopy of 2 FORMS OF IDENTIFICATION such as a copy of your motor vehicle license, voter registration, passport, school ID, county ID, vehicle registration, vehicle insurance card, green card, utility bill, or other form approved by the Registrar of the State of New Jersey.
  5. Mail your request to the Dover Health Department at the address at the top of the page.

MARRIAGE & REMARRIAGE CERTIFICATES

 

If you were married or remarried in the Town of Dover, Morris County, and wish to obtain a copy of your marriage certificate:

You must submit a completed application for a marriage or remarriage certificate accompanied by a Money Order made payable to the "Town of Dover" for $25.00 per copy.  The application form is available by clicking "application".
 

Enclose the following information with your marriage or remarriage certificate application:

 

  1. Telephone number.
  2. Self-addressed, stamped envelope.
  3. $25 Money Order payable to the "Town of Dover".  Please do not send cash in the mail.
  4. A photocopy of 2 FORMS OF IDENTIFICATION such as a copy of your motor vehicle license, voter registration, passport, school ID, county ID, vehicle registration, vehicle insurance card, green card, utility bill, or other form approved by the Registrar of the State of New Jersey.
  5. Mail your request to the Dover Health Department at the address at the top of the page.

DEATH CERTIFICATES

 

Persons eligible to request a death certificate:

 

1.      An executor, administrator of the estate or authorized representative of the decedent (must have a notarized letter from the informant, spouse or executor.

2.      Surviving spouse or caretaking partner.

3.      If there is no spouse or caretaking partner, another authorized member of the family can request a death certificate. An authorized member of the family means direct lineage including brother, sister, grandparents, grandchildren of the deceased and decedents children that are of legal age.

4.      A parent, guardian or other individual authorized under State law to act in a minor's behalf. This means that a person formally married or not married to the deceased but has children with them is entitled to the death certificate as long as they show their child's birth certificate with the deceased person's name on it.

5.      Funeral Director of record, responsible for the funeral or disposition of the body.

6.      A "significant other" may be authorized in writing by the other party to obtain a death certificate provided the application and required identification is supplied.

7.      An ex spouse may obtain a copy of the death certificate of the deceased former spouse when the record is being obtained on behalf of a minor child that was the product of the marriage. Documentary proof of the minor child and the need for the certificate must be provided.

 

If the death occurred in the Town of Dover, Morris County, and you wish to obtain a certified original copy of the death certificate, two (2) forms must be completed. You must submit a completed  "application" for a certified copy of a birth, marriage, death record and a completed "death certificate authorization form." Please note that the death certificate authorization form must be signed and notorized for your request to be processed by mail.

 

The "application" for a certified copy of a birth, marriage, death record form is available online by clicking "application."

Both the death certificate authorization form and the application must be accompanied by a Money Order made payable to the "Town of Dover" for $25.00 per copy.

When applying for a death certificate you must provide proof of relationship to the deceased indicating that you are the parent, spouse, grandchild, sibling, legal representative, or legal guardian.
 

Enclose the following information with your two (2) completed forms:

 

  1. Telephone number.
  2. Self-addressed, stamped envelope.
  3. $25 Money Order payable to the "Town of Dover".  Please do not send cash in the mail.
  4. A photocopy of 2 FORMS OF IDENTIFICATION such as a copy of your motor vehicle license, voter registration, passport, school ID, county ID, vehicle registration, vehicle insurance card, green card, utility bill, or other form approved by the Registrar of the State of New Jersey.

DOMESTIC PARTNERSHIPS - CIVIL UNIONS

 

If you registered your Domestic Partnership or Civil Union in the Town of Dover, Morris County, and wish to obtain a copy of your domestic partnership certificate, please enclose the following information with your request:

 

1.      A completed application for a Domestic Partnership or Civil Union Certificate. Please click "application" and use the application for a certified copy of a birth, marriage, or death record. Complete the applicant and marriage sections of the form.

2.      Telephone number.
